• Free health screenings such as blood pressure and A1c(blood sugar)
• Access to healthy foods and recipes to improve your diet
• Virtual and in-person support
• No insurance needed for services

Many people struggle to find the right healthcare resources or make lasting lifestyle changes. That's why everything we offer is available at no cost - thanks to generous community support through the Martha Jefferson Hospital Foundation.

Our team has helped hundreds of local residents prevent and manage chronic conditions like diabetes, heart disease, and obesity - and we're ready to help you, too.
